THYROID IN HUMAN BEINGS:- A STAGE




:-thyroid is a butterfly-shaped gland that sits low on the front of the neck. Your thyroid lies below your Adam’s apple, along the front of the windpipe. The thyroid has two side lobes, connected by a bridge (isthmus) in the middle. When the thyroid is its normal size, you can’t feel it.
The thyroid secretes several hormones, collectively called thyroid hormones. The main hormone is thyroxine, also called T4. Thyroid hormones act throughout the body, influencing metabolism, growth and development, and body temperature. During infancy and childhood, adequate thyroid hormone is crucial for brain development.





Thyroid Conditions

  • Goiter: A general term for thyroid swelling. Goiters can be harmless, or can represent iodine deficiency or a condition associated with thyroid inflammation called Hashimoto’s thyroiditis.
  • Thyroiditis: Inflammation of the thyroid, usually from a viral infection or autoimmune condition. Thyroiditis can be painful, or have no symptoms at all.
  • Hyperthyroidism: Excessive thyroid hormone production. Hyperthyroidism is most often caused by Graves disease or an overactive thyroid nodule.
  • Hypothyroidism: Low production of thyroid hormone. Thyroid damage caused by autoimmune disease is the most common cause of hypothyroidism .
  • Graves disease: An autoimmune condition in which the thyroid is overstimulated, causing hyperthyroidism.
  • Thyroid cancer: An uncommon form of cancer, thyroid cancer is usually curable. Surgery, radiation, and hormone treatments may be used to treat thyroid cancer.
  • Thyroid nodule: A small abnormal mass or lump in the thyroid gland. Thyroid nodules are extremely common. Few are cancerous. They may secrete excess hormones, causing hyperthyroidism, or cause no problems. 
  • Thyroid storm: A rare form of hyperthyroidism in which extremely high thyroid hormone levels cause severe illness.





Thyroid Tests

  • Anti-TPO antibodies: In autoimmune thyroid disease, proteins mistakenly attack the thyroid peroxidase enzyme, which is used by the thyroid to make thyroid hormones.
  • Thyroid ultrasound: A probe is placed on the skin of the neck, and reflected sound waves can detect abnormal areas of thyroid tissue.
  • Thyroid scan: A small amount of radioactive iodine is given by mouth to get images of the thyroid gland. Radioactive iodine is concentrated within the thyroid gland. 
  • Thyroid biopsy: A small amount of thyroid tissue is removed, usually to look for thyroid cancer. Thyroid biopsy is typically done with a needle. 
  • Thyroid stimulating hormone (TSH): Secreted by the brain, TSH regulates thyroid hormone release. A blood test with high TSH indicates low levels of thyroid hormone (hypothyroidism), and low TSH suggests hyperthyroidism.
  • T3 and T4 (thyroxine): The primary forms of thyroid hormone, checked with a blood test. 
  • Thyroglobulins: A substance secreted by the thyroid that can be used as a marker of thyroid cancer. It is often measured during follow-up in patients with thyroid cancer. High levels indicate recurrence of the cancer.




Thyroid Treatments

  • Thyroid surgery (thyroidectomy): A surgeon removes all or part of the thyroid in an operation. Thyroidectomy is performed for thyroid cancer, goiter,  or hyperthyroidism. 
  • Antithyroid medications: Drugs can slow down the overproduction of thyroid hormone in hyperthyroidism. Two common antithyroid medicines are methimazole and propylthiouracil. 
  • Radioactive iodine: Iodine with radioactivity that can be used in low doses to test the thyroid gland or destroy an overactive gland. Large doses can be used to destroy cancerous tissue.
  • External radiation: A beam of radiation is directed at the thyroid, on multiple appointments. The high-energy rays help kill thyroid cancer cells. 
  • Thyroid hormone pills: Daily treatment that replaces the amount of thyroid hormone you can no longer make. Thyroid hormone pills treat hypothyroidism, and are also used to help prevent thyroid cancer from coming back after treatment. 
  • Recombinant human TSH: Injecting this thyroid-stimulating agent can make thyroid cancer show up more clearly on imaging tests.

  • T3,T4 and TSH results

The T4 test and the TSH test are the two most common thyroid function tests. They’re usually ordered together.
The T4 test is known as the thyroxine test. A high level of T4 indicates an overactive thyroid (hyperthyroidism). Symptoms include anxiety, unplanned weight loss, tremors, and diarrhea. Most of the T4 in your body is bound to protein. A small portion of T4 is not and this is called free T4. Free T4 is the form that is readily available for your body to use. Sometimes a free T4 level is also checked along with the T4 test.
The TSH test measures the level of thyroid-stimulating hormone in your blood. The TSH has a normal test range between 0.4 and 4.0 milli-international units of hormone per liter of blood (mIU/L).
If you show signs of hypothyroidism and have a TSH reading above 2.0 mIU/L, you’re at risk for progressing to hypothyroidism. Symptoms include weight gain, fatigue, depression, and brittle hair and fingernails. Your doctor will likely want to perform thyroid function tests at least every other year going forward. Your doctor may also decide to begin treating you with medications, such as levothyroxine, to ease your symptoms.
Both the T4 and TSH tests are routinely performed on newborn babies to identify a low-functioning thyroid gland. If left untreated, this condition, called congenital hypothyroidism, can lead to developmental disabilities.

T3 results

The T3 test checks for levels of the hormone triiodothyronine. It’s usually ordered if T4 tests and TSH tests suggest hyperthyroidism. The T3 test may also be ordered if you’re showing signs of an overactive thyroid gland and your T4 and TSH aren’t elevatedTrusted Source.
The normal range for the T3 is 100–200 nanograms of hormone per deciliter of blood (ng/dL). Abnormally high levels most commonly indicate a condition called Grave’s disease. This is an autoimmune disorder associated with hyperthyroidism.

T3 resin uptake results

A T3 resin uptake, also known as a T3RU, is a blood test that measures the binding capacity of a hormone called thyroxin-binding globulin (TBG). If your T3 level is elevated, your TBG binding capacity should be low.
Abnormally low levels of TBG often indicate a problem with the kidneys or with the body not getting enough protein. Abnormally high levels of TBG suggest high levels of estrogen in the body. High estrogen levels may be caused by pregnancy, eating estrogen-rich foodsobesity, or hormone replacement therapy.


If your blood work suggests that your thyroid gland is overactive or underactive, your doctor may order a thyroid uptake test or an ultrasound test. These tests will check for structural problems with the thyroid gland, thyroid gland activity, and any tumors that may be causing problems. Based on these findings, your doctor may want to sample tissue from the thyroid to check for cancer.
If the scan is normal, your doctor will likely prescribe medication to regulate your thyroid activity. They will follow up with additional thyroid function tests to make sure the medication is working.



Hypothyroidism occurs when levels of the two thyroid hormones, triiodothyronine (T3) and thyroxine (T4), are too low. Although changing your diet alone isn’t enough to restore normal thyroid hormone levels, avoiding some foods and eating more of others can improve your body’s absorption of these hormones.

Foods to avoid

Many common foods and supplements contain compounds that interfere with thyroid function. In general, it’s best to avoid the following:

Soy

Studies suggest that phytoestrogens in soybeans and soy-rich foods may inhibit the activity of an enzyme that makes thyroid hormones. One study found that women who consumed soy supplements were three times more likely to develop hypothyroidism.

Iodine-rich foods

Some forms of hypothyroidism are caused by a lack of sufficient iodine. In such cases, using iodized salt or iodine-enriched foods can be beneficialTrusted Source. But eating too much iodine can have the opposite effect and suppress thyroid gland activity. Check with your doctor before taking supplements.

Iron and calcium supplements

Taking iron or calcium supplements can also change the effectiveness of many thyroid medications.

High-fiber foods

Although a high-fiber diet is usually recommended, too much fiber eaten right after taking thyroid medicines may interfere with their absorption. Wait two hours before you eat a high-fiber meal (one with more than about 15 grams of fiber).

Certain vegetables

Cruciferous vegetables that are rich in fiber, like broccoli, cabbage, spinach, kale, and Brussels sprouts, may inhibit thyroid medication absorption. Reducing the amounts of such produce in the morning right after taking your medication may help.
Caffeine, tobacco, and alcohol can also influence the effectiveness of thyroid medicine. Ask your doctor for tips on how to regulate or reduce your consumption.

Foods to eat:-
Nutrient-rich foods that improve your health may also benefit your thyroid gland. Certain compounds and supplements may help as well. These include:

Antioxidant-rich fruits and vegetables

Blueberries, tomatoes, bell peppers, and other foods rich in antioxidants can improve overall health and benefit the thyroid gland. Eating foods high in B vitamins, like whole grains, may also help.

Selenium

Tiny amounts of selenium are needed for the enzymes that make thyroid hormones to work properly. Eating selenium-rich foods, such as sunflower seeds or Brazil nuts, can be beneficial.

Tyrosine

This amino acid is used by the thyroid gland to produce T3 and T4. Good sources of tyrosine are meats, dairy, and legumes. Taking a supplement may help, but ask your doctor beforehand.
